Each service rendered during the office visit must be documented appropriately to prevent third-party payer’s denial of services because of a lack of information that proves medical necessity. Keep in mind, progress in resolving the functional deficit shows the chiropractic patient’s progress and is required to tell the story of the medical necessity in the routine office visit.
